The majority of hospice care is provided in the patient’s home, with family and friends acting as caregivers. If there is not a residence, the agency can provide care in a Nursing facility or other type of residential facility.

If a patient requires inpatient care in order to address severe symptoms, or to provide respite for a caregiver, we have relationships with hospitals and Nursing facilities in our service area, allowing patients to be served in their own communities.

We have a triage team available 24 hours a day, seven (7) days a week. When you call in the evening times or on weekends, we will always be available, even if it is not medical in nature. If an evening or weekend visit is needed, you can count on us to be there for you.

Achieve acceptable pain control on all patients. We emphasize the delivery of pain management in a timely and caring manner
Respond to all patient related calls within 20 minutes of receiving your call
